Project EMPOWER

Project EMPOWER is a free program of the Sinclair Health Clinic striving to Empower a Healthy Community utilizing Community Health Workers (CHWs) to:

Connect program participants to community resources

Support in social determinants of health
(i.e. employment, housing, transportation, access to food and needed 
social services)

Educate regarding chronic disease prevention and healthy living

Every client will be EMPOWERED . . .

1.

To assume inherent dignity.

2.

To access and utilize community and government resources for improved health.

3.

To develop social determinants of health.

4.

To know and manage individual health indicators

Clients engage in weekly visits with their CHW- either in their home, at the clinic, or at a location of the client’s choosing (ex. public library)- working towards goals the client sets with their CHW aiming towards improved health and wellness.

Project EMPOWER is coordinated by a Registered Nurse who assists with the client’s plan of care and medication reconciliation with the goal of increasing treatment adherence and improving overall health and well-being.
